Higashi-Matsusaka Station opened on 27 March 1930 with the Sangu Express Electric Railway's inaugural run between Matsusaka and Geku-mae, now Miyamachi. Through the 15 March 1941 merger with Osaka Electric Railway, the station joined Kansai Express Railway and became part of its Yamada Line; the 1 June 1944 consolidation with Nankai Electric Railway placed it on Kintetsu. Operations were suspended on 1 June 1945 and resumed on 15 March 1946. PiTaPa acceptance began on 1 April 2007, the station was destaffed around the clock from 21 December 2013, and a new platform-1 entrance opened on 21 February 2014 in place of the on-site level crossing between platforms.

Higashi-Matsusaka sits 3.9 kilometres from neighbouring Kushida Station, the longest gap between any two adjacent stations on the Yamada Line.
